The base is a ball-bearing roller stand (used for extra-man support off table saws and such) from Home Depot (about $20); model WM9-8.
Used an old Bogen 3D head with quick-release plate from a video tripod for the pivoting system.
Built the custom heli support from MDF and a pair of heli transport mounts I picked up over at Rick's (www.ronlund.com, part #KSJ680).
The cam-clamps come from Rockler (www.rockler.com), part #58244
Attempt at creating an all-metal (aluminum) base employing the services of eMachineShop.com.
As you can see, job didn't turn out too good, mostly due to the thin material and probably fabrication method.
The "teal" color is supposed to be blue anodizing (compare to the cam clamps blue) ... looks like someone dunked the parts in cheap Rit dye.
Note all the cracks around the bend lines; one even can be seen through it. Very bad work, and not even a warning from the EMS software or fabricators.
Total parts/labor cost: $240.26 and took 36 days to get in.
